Your role as Care Coordinator
Dialogue is looking for a Care Coordinator (Overnight) to take part in our mission to help people improve their wellbeing. Working closely with the Team Lead of Care Coordinators and Operational Support Services teams, your role will be to act as a primary point of contact for our members to support operations during evenings & nights.

What you'll be doing
- Act as the first point of contact between patients and the medical team.
- Act as a liaison between medical and technical support teams to help patients and providers resolve various medical, technical, or logistical issues
- Assist patients in finding appropriate resources to deliver a a 5 star patient experience
- Manage crises, monitor operations during the night, and provide technical support
- Provide administrative assistance to clinicians
- Advise patients to help them navigate their province's medical system
- You will be responsible for patient safety in your member-facing role
- Working closely with the medical team to ensure overnight coverage
We'd love to hear from you if you have
- Personality traits: autonomy, attention to detail, inquisitive, positive attitude
- Available to work night shifts (from 10:30pm to 7am) on weekdays and weekends as part of our night team’s schedule rotation
- Ability to manage crises and solve problems quickly and calmly
- Good time management and ability to prioritize tasks effectively
- Customer service-oriented mindset and willingness to help others
- Motivated by a dynamic work environment, focused on the patient experience
- Excellent communication skills, both written and verbal
- Strong computer skills
- Experience in healthcare or administration and/or working night shifts is an asset
